Help take the anxiety out of your first appointment by filling out your forms beforehand in a calm and thoughtful atmosphere.
Before your appointment, we encourage you to take time to look at and fill out the two forms that are linked here so that you are not rushed or overwhelmed by paper when you come to your initial appointment: General Information Form and Informed Consent. You may bring this paperwork filled out to your first session or you may save it as a Word or pdf document and email it to me. The forms are linked to the boldfaced, underlined words.
If you have questions about what the form says or means, please come to your first session with those questions. Whether or not you have questions, we always go over the forms together at the first session.
General Information Form and Informed Consent: The first form is general information about you—your name, address, living situation, your general health, a few questions about your family of origin, and what is going on that you need some help with—what has brought you to me. Note to couples: Please have each person fill out a separate form.

Informed Consent: The second form in the General Information Form packet is about your rights as a client including HIPAA, confidentiality, and my record-keeping.

No Surprises Act/Good Faith Estimate: This notice is information regarding this legislation that became effective on January 1, 2022. This is a sample form that you will receive when making your first appointment.

Telemental Health Consent (Word): This form is your agreement to treatment via telemental health - phone or HIPAA compliant video conferencing.
Release of Information (Word or pdf ): A form you sign in order for Soul Spa Seattle, LLC to release information to a doctor, therapist, etc. in order to help further your healing so that all practitioners have a fuller understanding of your needs and goals.
